2025 RELEASE WAIVER

This form, completed in its entirety and signed by a parent/guardian is required for every child and student that participates in Parkway's children and student ministry events during the 2025 calendar year. Note, please attach copies of your medical insurance to the completed form prior to turning it in. If you need any help or have any questions, contact the church office for assistance.
